Policies & Procedures
Office Hours
All of our offices are open Monday through Friday from 8:00 a.m. until 4:00 p.m. Walk in Urgent Care is available at all locations during normal office hours. On Saturday, our Gateway office is open for walk in patients from 8:00 a.m. until noon.
Phone Calls
Please call our office at 812-424-9291 or 800-264-1208 if you have any questions regarding your condition, medication, or treatment. Our telephone is answered 24 hours a day, seven days a week, including holidays. Most incoming calls are handled by office staff, permitting physicians to care for patients with minimal interruptions. Urgent calls are returned as soon as possible and our intent is to return all calls within the same day a message is received.
Appointments

Cancellation Policy
24-hour notice is required for appointment cancellations. Some of our physicians charge a fee for new patients that "no call, no show" for an appointment.
Materials Required for Second-Opinion Appointments or New Patients with Previous Treatment
Any patient with previous treatment from another physician must bring all existing reports, films (both MRI and x-ray), and consult notes or progress notes. Second-opinion patients and new patients who have had prior treatment WILL NOT BE SEEN without all appropriate medical documentation of that treatment.
Prescription Refills
Prescription refill requests may be made Monday through Thursday from 8:00 a.m. to 3:00 p.m. We suggest that you give us 24 hours to refill any prescription you may need. This time period is recommended so that we may access your medical records and the prescription guidelines recommended by your managed care plan. No prescriptions will be refilled on weekends or holidays.

Insurance
Orthopaedic Associates accepts most insurance options. Please present your insurance card at the time of your visit. Please note that it is the patient's responsibility to know your insurance coverage. If you have questions about whether your insurance plan gives you access to our physicians, please call the phone number on the back of your insurance card. In many cases, insurance–including Medicare–won't cover an entire bill for service, so it's important that you not ignore bills.
Financial Arrangements and Collections
To enable you to take care of your financial obligations for your care, Orthopaedic Associates accepts Visa, MasterCard and Discover. If your account does become past due–90 days with no payment–and you have not arranged a satisfactory payment plan with us, we will take appropriate action to recover the balance.
Disability Forms
Orthopaedic Associates charges a fee for completing and signing forms certifying loss of work time for banks, finance companies, etc. To access a disability form that you can submit to an outside organization, please click here. You should complete as much of the form as possible prior to submitting it to OA for completion and signature.

Information Requests
Copies of your records will be provided upon request to you, physicians, attorneys, legal representative or agents, and insurance carriers for a pre-paid fee and will be mailed within 10 working days of the request.

X-ray Policy
X-rays taken at Orthopaedic Associates become part of our permanent record of your care. Their cost includes personnel services, equipment and film use, and interpretation by our physicians. You will be charged an additional fee for x-ray copies.
Hospitals
For inpatient care, patients most often are admitted to Deaconess Hospital, Deaconess Gateway Hospital, Methodist Hospital, St. Mary's Hospital, and St. Mary's Warrick Hospital.
Inpatient and Emergency Room Fees
Your insurance carrier will be billed by this office for inpatient or emergency room services performed by your physician. These charges are separate from the hospital's fees and will not be reflected on your hospital statement.
